How do I pay for my service? What are acceptable forms of payment?
The service can be payed for in advanced or upon arrival. The form of payments we accept are cash, cash app, credit, or debit. The fee charged by magic labs mobile phlebotomy is strictly for the services of the blood draw. If you have insurance your provider will cover the cost of your tests with the Lab that is convenient for you or assigned by your provider. If you do not have insurance you will be responsible for the fees incurred by the chosen lab. If this is a facility please contact us for rates.
What if I am not satisfied with Magic labs mobile phlebotomy?
Our services are guaranteed and in the event that you are dissatisfied you will be refunded.
What documents do I need for my appointment.
Please have your requisition (doctors order), a copy of your valid identification, and a copy of your insurance card front and back if you are using insurance for your lab fees. If you do not have a doctors order we can still get you drawn through our partnership with ulta labs.
